I can't seem to find the 48-sided garnet, which is an upgrade component of the Holy Avenger and the Wounding Bastard sword. I have completed every encounter possible in SoA save the fight with Demogorgon and the fight after forging the Judgment Day Sword. Any hints on where it may be found?

Mm, i got only the elven city suldeneselar left to do in SoA and still haven't found this gem.Could someone give me a hint?
